Endocrinology: The ovaries and other reproductive organs are in continuous interaction with the pituitary and the thyroid. The interplay may account for infertility occurring at different levels and may render the diagnosis difficult. Thyroid disorders, both hyperthyroidism and hypthyroidism, can interact with the ovaries, through a direct effect on the and indirect through alterations of the SHBG levels, the PRL levels and the coagulation factors, important for normal menstruations. Data in literature on the prevalence of thyroid disorders (dysfunction and autoimmunity) in infertile women are well-documented. In case of infertility due to polycystic ovarian syndrome and idiopathic infertility, a higher prevalence of thyroid autoimmunity (TAI) has been documented compared to that in fertile women. The preparation for an assisted reproductive technology (ART), the so-called controlled ovarian hyperstimulation (COH) is known to impact on normal thyroid function, highlighting the bilateral interaction between both types of hormones. Furthermore it is known, that women pregnant after ART with TAI, carry a significant higher risk for a first trimester miscarriage, compared to women without TAI.
