Abstract

Stability of blood eosinophils in stable Chronic Obstructive Pulmonary Disease: a retrospective study in primary care.



Introduction

Blood eosinophils as a biomarker for the guidance of chronic treatment of Chronic Obstructive Pulmonary Disease (COPD) were recently included in the 2019 Global Initiative for Obstructive Lung Disease guideline. Given their easy accessibility and the possible link between eosinophil count and response to use of inhaled corticosteroids, blood eosinophil counts can be an interesting tool in daily practice. However, the stability over time of this biomarker remains unclear.

Aims and objectives

A retrospective analysis of the intra-patient variability of blood eosinophil counts in patients with COPD in a stable state.

Methods

A retrospective study on data gathered from the Electronic Health Record of COPD patients from two primary care practices in Belgium was conducted. All eosinophil measurements of these patients recorded outside exacerbation periods were included. Patients with current asthma or less than two eosinophil counts were excluded. For each patient the relative standard deviation (RSD) of the eosinophil counts was calculated to characterise the intra-patient variability.

Results

73 patients were included, yielding 913 eosinophil measurements, between 0 and 1046 cells/µL. The average eosinophil count per patient ranged between 15 and 507 cells/µL with an evenly distributed average RSD per patient of 0,50.

Conclusion

The large RSD values recorded, imply that blood eosinophil counts are highly variable in individual COPD patients. Therefore the use of a single measurement to guide therapeutic decisions remains debatable.
